What Are Orthobiologic Treatments?
Orthobiologics are substances derived from biological sources that may be used as part of the treatment of selected bone, joint, tendon, ligament, and other musculoskeletal conditions.
You may also hear terms such as “biologic treatment” or “regenerative medicine.”
These terms cover several very different products and procedures, and they should not be assumed to work in the same way or have the same evidence.
At Arthritis & Sports, our approach is to consider orthobiologic treatments only after determining the underlying diagnosis and reviewing whether available research supports their use for that particular condition.
The goal is not to promote a treatment simply because it is new or described as regenerative.
The goal is to choose the treatment that makes the most sense for the individual patient.
Why Does the Diagnosis Matter?
Orthopedic pain can come from many different structures.
Knee pain may result from arthritis, a meniscus injury, ligament damage, cartilage injury, tendon problems, or another condition.
Shoulder pain may originate from the rotator cuff, biceps tendon, joint arthritis, instability, or surrounding tissues.
Because these conditions are different, the same injection should not automatically be offered for all of them.
Before considering an orthobiologic treatment, your orthopedic provider may review your symptoms, examination, imaging, previous treatments, activity goals, and overall health.
That evaluation helps determine whether an orthobiologic treatment has a reasonable role—or whether another treatment has stronger evidence.
Where Do Orthobiologics Fit Into Non-Surgical Orthopedic Care?
Orthobiologic treatments are usually one option within a much broader nonsurgical treatment strategy.
Depending on the condition, treatment may begin with:
physical therapy and exercise
temporary activity modification
medication when appropriate
bracing, splinting, or orthotics
traditional injections
or another diagnosis-specific treatment
An orthobiologic procedure may be considered when the diagnosis and available evidence support its use and the expected benefits, limitations, risks, and costs have been discussed.

What Orthobiologic Treatments Are Used in Orthopedics?
Several different products and procedures are commonly discussed under the broad category of orthobiologics.
They are not equivalent.
Research, regulation, preparation methods, and clinical evidence vary greatly among them.
Platelet-Rich Plasma
Platelet-rich plasma, commonly called PRP, is prepared from a sample of the patient's own blood.
PRP is one of the more commonly studied orthobiologic treatments in orthopedics and may be considered for selected musculoskeletal conditions.

Other Products Marketed as Regenerative Medicine
Patients may encounter advertisements for products described as stem cells, exosomes, amniotic products, umbilical-cord products, or other regenerative therapies.
These products are very different from PRP.
Patients should be cautious about claims that these treatments have been proven to regrow cartilage, reverse arthritis, rebuild damaged joints, or reliably eliminate the need for surgery.
The regulatory status and evidence for a specific product should be understood before treatment is considered.
What About Stem Cell Injections?
The term “stem cell therapy” is frequently used in orthopedic marketing, but patients should understand the regulatory limitations.
The U.S. Food and Drug Administration states that currently approved stem-cell products in the United States are blood-forming stem cells used for certain blood-related disorders.
The FDA states that stem-cell and similar regenerative products have not been approved for orthopedic conditions such as osteoarthritis, tendonitis, disc disease, or joint and spine pain.
For that reason, Arthritis & Sports does not recommend describing unapproved orthopedic injections as proven stem-cell therapies or making claims that they regenerate damaged joints.
Patients considering any product marketed as stem-cell therapy should ask exactly what is being injected, where the product comes from, what FDA status applies, and what high-quality evidence supports its use.
Can Orthobiologic Treatments Regrow Cartilage?
Patients should be cautious with claims that an injection will regrow cartilage or return an arthritic joint to its original condition.
Some orthobiologic treatments have shown improvements in pain or function for selected conditions.
Improved symptoms are not the same as proving that damaged cartilage, tendons, or ligaments have been fully regenerated.
For osteoarthritis in particular, the goal of treatment is often to manage symptoms and improve function.
When arthritis becomes advanced, joint replacement or another surgical treatment may ultimately be more appropriate.

Can Orthobiologic Treatments Help Avoid Surgery?
Sometimes a nonsurgical treatment may provide enough improvement that surgery is not currently necessary.
That does not mean an orthobiologic injection should be described as a guaranteed alternative to surgery.
Whether surgery is needed depends primarily on the diagnosis and severity of the problem.
A chronic tendon condition may have very different treatment options from a complete tendon rupture.
Mild or moderate arthritis may be managed nonsurgically for years, while advanced joint damage may eventually require replacement.
The goal should be to use the appropriate treatment—not simply to avoid surgery at all costs.
What Does the Evidence Say About Orthobiologics?
Orthobiologic research continues to evolve.
Evidence is stronger for some treatments and diagnoses than for others.
The effectiveness of a biologic procedure can also be influenced by how the product is prepared, which tissue is treated, severity of the condition, rehabilitation, and patient characteristics.
For this reason, statements such as “regenerative injections heal orthopedic injuries” are too broad.
A more useful question is: What evidence supports this particular treatment for my particular diagnosis?

How Are Orthobiologic Treatments Different From Cortisone?
Orthobiologic and corticosteroid injections are different categories of treatment.
Corticosteroids are medications used to reduce inflammation and may provide temporary symptom relief for selected orthopedic conditions.
Orthobiologic treatments use substances derived from biological sources and are intended to interact differently with the local tissue environment.
Neither category is universally better.
The appropriate option depends on the condition being treated, location, prior care, timing, patient health, and individual goals.

Are Orthobiologic Treatments Covered by Insurance?
Insurance coverage varies significantly.
Some orthobiologic treatments are considered investigational or are not routinely covered by health insurance.
Coverage can depend on the treatment, diagnosis, carrier, and individual insurance plan.
Before undergoing treatment, patients should understand both the clinical recommendation and any expected out-of-pocket expense.
What Questions Should I Ask Before Having an Orthobiologic Treatment?
Patients considering any biologic or regenerative treatment should understand exactly what is being recommended.

How Does Physical Therapy Fit Into Orthobiologic Care?
An injection does not restore strength, mobility, endurance, coordination, or movement mechanics by itself.
For many tendon, sports, and musculoskeletal conditions, rehabilitation remains an important part of treatment.
Physical therapy may be used before or after a procedure to improve movement, progressive loading, strength, and return to activity.
The timing and intensity of rehabilitation depend on the diagnosis and treatment performed.
