Scheduling Your Appointment
For Orthopaedic Appointments:
Please call the receptionist at 703-444-5000, Monday through Friday 8:00 a.m. to 4:30 p.m.
For Physical Therapy Apointments:
Please call the receptionist at 703-433-2500 Monday through Thursday 7:00 a.m. to 7:00 p.m, Friday 7:00 a.m. to 5:00 p.m.

We understand your time is in valuable. To ensure adequate time is scheduled for your appointment please be prepared to provide the following information to the receptionist:
- Your name
- Your date of birth
- Your address
- Primary and secondary phone numbers
- Brief description of the reason for the visit
- Your primary physician or health care provider
- Your insurance or HMO information

Once Your Appointment Is Scheduled
Please bring the following items with you to every visit:
- Driver's License or Other Valid Government-Issued Photo Id
- Insurance card and any co-payment due for the visit
- Referral forms (if applicable)
- X-rays and any related studies (if applicable)
- Any necessary new patient or follow-up forms

Late Arrivals And Cancellations
If this is your first appointment, please arrive 20 minutes early to fill out any necessary paper work. Please be advised, patients who arrive more than 15 minutes after their scheduled appointment time may need to be rescheduled. If you are unable to keep an appointment or need to reschedule, please call us at least 24 hours in advance. We look forward to your visit!

The MultiPlan Network is a PPO (Preferred Provider Organization) network that is accessed by some insurance companies as part of the health plans they offer to members. What does this mean for you? Our services may now be available to you through your health plan at a discounted rate because we now participate in the MultiPlan Network. This is a benefit to our patients because we are now able to accept a wider range of insurances. How do you know if you have access to the MuliPlan Network through your health plan?
